ABSTRACT

La autopsia médico legal en Costa Rica, en casos sospechosos de intoxicación por cocaetileno se debe realizar bajo las normas establecidas en la Guía de estándares de trabajo para la Sección de Patología Forense del Departamento de Medicina Legal. El análisis del mecanismo fisiopatológico de cómo estas sustancias provocan alteraciones en el organismo que pueden conllevar a un eventual fallecimiento corresponde a parte del análisis requerido en la investigación ante la sospecha de esta causa de muerte. Por lo anterior, el objetivo de este artículo es describir los mecanismos fisiopatológicos que ocurren durante el consumo combinado de cocaína y etanol, los mecanismos que conllevan a la muerte de personas consumidoras de estas sustancias y las consideraciones médico legales a tomar en cuenta para el diagnóstico de esta causa de muerte. Se realizó revisión de artículos científicos, sobre los efectos del uso combinado de la cocaína y el etanol. La literatura describe que el uso combinado de cocaína y etanol potencia los efectos farmacocinéticos y bioquímicos de cada una de estas sustancias, que su derivado, el cocaetileno, es capaz de generar por sí mismo los mecanismos causantes de la muerte. Que los principales mecanismos fisiopatológicos que conllevan la muerte ante el uso combinado de estas sustancias son de origen cardiovascular y hepático. Como consideraciones médico legales a tomar en cuenta para el diagnóstico de esta manera de muerte accidental, en la Sección de Toxicología del Departamento de Ciencias Forenses de Costa Rica, la cuantificación del cocaetileno y las sustancias relacionadas no se realiza, aunque se encuentra actualmente en el desarrollo de un proyecto para la determinación de la estabilidad de las drogas en sangre bajo las condiciones de almacenamiento, con el fin de ofrecer la posibilidad de cuantificar ciertas drogas (en donde se podría incluir el cocaetileno) en un futuro próximo.


Medical-legal autopsy in Costa Rica, in suspected cases of cocaethylene poisoning must be performed under the regulations established in the Work Standards Guide for the Forensic Pathology Section of the Department of Legal Medicine. The analysis of the pathophysiological mechanism of how these substances cause alterations in the organism that can lead to eventual death corresponds to part of the analysis required in the investigation when this cause of death is suspected. Therefore, the objective of this article is to describe the pathophysiological mechanisms that occur during the combined consumption of cocaine and ethanol, the mechanisms that lead to the death of people who consume these substances, and the medico-legal considerations to be considered for the diagnosis. of this cause of death. A review of scientific articles was carried out on the effects of the combined use of cocaine and ethanol. The literature describes that the combined use of cocaine and ethanol enhances the pharmacokinetic and biochemical effects of each one of these substances, that its derivative, cocaethylene, can generate the mechanisms that cause death by itself. That the main pathophysiological mechanisms that lead to death in the combined use of these substances are of cardiovascular and hepatic origin. As legal medical considerations to take into account for the diagnosis of this type of accidental death, in the Toxicology Section of the Department of Forensic Sciences of Costa Rica, the quantification of cocaethylene and related substances is not carried out, although it is currently in the development of a project for the determination of the stability of drugs in blood under storage conditions, in order to offer the possibility of quantifying certain drugs (which could include cocaethylene) in the near future.

ABSTRACT

O número de pessoas utilizando substâncias ilícitas de forma recreativa aumenta a cada ano, chamando a atenção de estudiosos de diversas áreas do conhecimento. Com isso, a demanda de exames toxicológicos exigida para trabalhadores, vítimas de crimes e esportistas também tem crescido. A amostra biológica mais utilizada para análises toxicológicas continua sendo a urina, visto que sua obtenção é menos invasiva, possibilita coletar grande volume de amostra e pode-se detectar substâncias até dias após ter ocorrido a exposição ou consumo. Entretanto, estas amostras necessitam de um grande volume físico para serem armazenadas e transportadas aos laboratórios, devendo ser mantidas em temperatura baixa e controlada para conservação. Outro ponto a se considerar é a quantidade de amostra insuficientemente coletada, ou extravasamento do conteúdo, contaminando outras amostras e muitas vezes, inviabilizando a análise. Uma alternativa recente para tais problemas é utilizar a técnica chamada de dried urine spots (DUS), onde poucos microlitros de urina são colocados em um papel absorvente e secos sob temperatura ambiente, preservando de agentes degradantes os componentes presentes na urina. Assim, o objetivo deste trabalho é avaliar a estabilidade das substâncias do presente estudo em alta temperatura, temperatura ambiente e em temperaturas de 4°C e -20°C. Para este fim, foi necessário desenvolver, validar e aplicar métodos de extração e determinação de anfetaminas e produtos de biotransformação de cocaína e tetraidrocanabinol carboxílico (THCCOOH) em amostras dried urine spot, utilizando cromatografia líquida acoplada à espectrometria de massas. Os picos foram identificados por UPLC-ESI-MS/MS, com tempo total de 5 mins utilizando fase A- água, formiato de amônio e 0,1% ácido fórmico, e B- metanol: acetonitrila (6:4) + 0,1% de ácido fórmico. A extração foi feita utilizando acetonitrila: metanol: acetona (1:1:1) +ácido fórmico 0,1%. Não foi possível iniciar a validação de THCCOOH, visto uma possível complexação do analito com o papel. Para as outras substâncias, o método cromatográfico desenvolvido se mostrou eficiente e seletivo, com LOD e LOQ de 10 ng/mL para todos os analitos, sendo linear até 1000 ng/mL, atendeu as especificações de precisão e exatidão e carryover. As amostras permaneceram estáveis ao longo de 32 dias nas temperaturas estudadas, demonstrando a segurança em se utilizar a técnica de DUS para armazenamento e transporte de amostras biológicas dentro da faixa de temperatura do estudo até 32 dias


The number of people using illegal substances in a recreational way increases each year, drawing the attention of scholars from different areas of knowledge. As a result, the demand for workplaces drug tests, toxicological tests for victims of crimes and dopping has also grown. The biological sample most used for toxicological tests remains urine, since obtaining it is less invasive, it is possible to collect a large volume of sample and it is possible to detect substances up to days after exposure or consumption has occurred. However, these samples require a large physical volume to be stored and transported to the laboratories, and must be kept at a low temperature for conservation. Another point to consider is the amount of sample insufficiently collected, or leakage of the content, causing contamination of other samples and often making the analysis unfeasible. A recent alternative to such problems is to use "dried urine spots" (DUS), where few microliters of urine are placed on absorbent paper and dried at room temperature, preserving the components present in the urine from degrading agents. Thus, the objective of this work is to evaluate the stability of the substances in this study at high temperature, room temperature and at temperatures of 4°C and -20°C. For this purpose, it was necessary to develop, validate and apply methods of extraction and determination of amphetamines and biotransformation products of cocaine and carboxylic tetrahydrocannabinol (THCCOOH) in dried urine spot samples, using liquid chromatography coupled to mass spectrometry (LC-MS). The peaks were identified liquid chromatography coupled to a mass spectrometer (UPLC-ESI-MS/MS), with a total time of 5 mins using phase A- water, ammonium formate and 0.1% formic acid, and B- methanol: acetonitrile (6:4) + 0.1% formic acid. Extraction was done using acetonitrile: methanol: acetone (1:1:1) + 0.1% formic acid. It was not possible to perform the validation of THCCOOH, given a possible complexation of the analyte with the paper. To the others substances, the chromatographic method developed proved to be efficient and selective, with LOD and LOQ of 10 ng/mL for all analytes, being linear up to 1000 ng/mL, meeting the specifications of precision and accuracy and carryover. The samples remained stable for 32 days at the temperatures studied, demonstrating the safety of using the DUS technique for storage and transport of biological samples until 32 days on temperature range studied

RESUMO: Introdução: O uso da cocaína é considerado um importante problema de saúde, sendo crescente o número de atendimentos nos serviços de emergência de pacientes que referem sintomas relacionados a alterações cardiovasculares após seu uso. Objetivo: Apresentar as repercussões cardiovasculares promovidas pela cocaína, associando o seu consumo a possíveis desfechos fatais, além de discutir as melhores maneiras de abordar o paciente na emergência. Método: Revisão integrativa da literatura nas bases de dados eletrônicas LILACS, MEDLINE, IBECS, SciELO, Secretaria Municipal de Saúde de São Paulo, Secretaria Estadual de Saúde de São Paulo, Coleciona SUS, disponíveis na BVS, bem como no PubMed. Resultados: Após análise criteriosa, 44 artigos atenderam aos critérios de inclusão. Foram estudos realizados na Europa (25%), Austrália (6,82%) e no continente americano (68,18%). Discussão: Abuso de cocaína é considerado um fator de risco isolado para doenças cardiovasculares, independente de outros. Por isso, pacientes jovens admitidos com dor torácica devem ser questionados sobre o consumo dessa droga. O tratamento do IAM associado ao uso de cocaína é semelhante ao realizado em paciente não usuário e, apesar da controvérsia existente sobre existir ou não contraindicação ao emprego de betabloqueador, diversos estudos indicam a utilização de drogas alfa e beta bloqueadores nesses pacientes. Conclusão: constatou-se que as principais repercussões cardiovasculares encontradas em pacientes usuários de cocaína que procuraram atendimento nas emergências hospitalares foram as emergências/urgências hipertensivas, taquicardia supraventricular, angina, infarto agudo do miocárdio, dissecção e aneurisma de aorta. ABSTRACT: Introduction: The use of cocaine is considered an important health problem. An increasing number of patients in the emergency services report symptoms related to cardiovascular changes after its use. Objective: To present the cardiovascular repercussions promoted by cocaine, associating its consumption with possible fatal outcomes, besides discussing the best ways to approach the patient in the emergency. Method: Integrative literature review in the electronic databases LILACS, MEDLINE, IBECS, SciELO, Sao Paulo Municipal Health Department, State Health Secretariat of São Paulo, Coleciona UHS, available at the VHL, as well as PubMed. Results: After careful analysis, 44 articles met the inclusion criteria. Studies were conducted in Europe (25%), Australia (6.82%) and the American continent (68.18%). Discussion: Cocaine abuse is considered an isolated risk factor for cardiovascular disease, independent of others. Therefore, young patients admitted with chest pain should be asked about the use of this drug. The treatment of AMI associated with cocaine use is similar to that performed in a non-user patient. Despite the controversy over whether or not there is a contraindication to the use of beta-blockers; several studies indicate the use of alpha and beta-blocker drugs in these patients. Conclusion: it was found that the main cardiovascular repercussions found in cocaine users who sought care in hospital emergencies were hypertensive emergencies/urgencies, supraventricular tachycardia, angina, acute myocardial infarction, dissection, and aortic aneurysm (AU)

ABSTRACT

Abstract Pyoderma gangrenosum associated to the use of cocaine/levamisole is a rare condition associated to their consumption. Cocaine use is frequent in Colombia, and the substance is contaminated with levamisole, an anthelmintic that increases the psychotropic effects and enhances its side effects. We present three clinical cases of patients with ulcerated lesions, in which the diagnosis was pyoderma gangrenosum secondary to the use of cocaine contaminated with levamisole. This called the attention of the health staff to investigate the abuse of substances in gangrenous pyoderma and also evidence that the interruption of consumption was the basis of management.

Abstract Introduction: Cocaine use is known to be associated with an increased risk for vascular diseases. It is likely to trigger or increase the risk for an aortic dissection. We conducted an analysis of 45 cases of cocaine-related aortic dissection to further characterize the clinical features and outcomes of this patient cohort. Methods: Our study cohort of 45 patients consisted of 11 cases from our institutional database and 34 published case reports. Results: The observed cases of acute aortic dissection related to cocaine use showed a high proportion of young (41.3±8.67 years) and male (88.9%) patients. Most of the cases (75%) were classified as Stanford type A. Also, in 75% of the cases, cocaine use was prevalent for more than one year. Median time from last cocaine use to onset of symptoms was one hour. In-hospital mortality was 21.4%, while additional 11.9% of the cases died before arriving at the hospital. Conclusion: Acute aortic dissection related to cocaine use occurs in predominantly young male patients and has a dismal outcome when compared to all comer series.

El abuso del consumo de cocaína puede ocasionar problemas físicos y mentales graves. Dicha droga puede ser utilizada de varias formas y sus efectos sobre la cavidad oral varían según la vía de administración, causando desde erosiones en las piezas dentarias, abrasiones cervicales, caries, enfermedad periodontal, disfunción temporomandibular, xerostomía, ulceraciones hasta la perforación del paladar duro y/o blando. Se describirá el caso clínico de un paciente cocainómano que concurrió al servicio de odontología del Hospital San Martin de La Plata, presentando dos perforaciones en el paladar duro debido al consumo crónico, y el tratamiento de urgencia correspondiente mediante una placa obturatríz que favorece la deglución y el habla del paciente (AU)

ABSTRACT

El consumo de cocaína y/o cannabis durante el embarazo constituye un problema en ascenso, de importancia para la salud pública mundial. Los niños expuestos pueden presentar un amplio rango de complicaciones en el período perinatal, pero los conocimientos sobre la evolución posterior son escasos.Objetivo: Describir y comparar las trayectorias sanitarias de niños expuestos y no expuestos prenatalmente a cocaína y/o cannabis durante 4 años. Métodos: Estudio de cohorte retrospectivo con grupo de comparación doble. Los niños expuestos fueron detectados en el Servicio de Neonatología de un hospital público mediante una prueba de orina, entre 2009 y 2013. Resultados: Se evaluaron 29 niños expuestos y 58 no expuestos. Las principales drogas detectadas en el grupo expuesto fueron cocaína y cannabis. La mayoría de las madres fueron policonsumidoras. En los niños del grupo expuesto, se encontraron diferencias significativas en menor frecuencia de controles de salud (p < 0,0001) y mayor frecuencia de consultas en Emergencias (p = 0,0295) e Internaciones (p = 0,007), principalmente, por cuadros respiratorios. Presentaron, además, mayor frecuencia de enfermedad pulmonar obstructiva crónica, cambios de hogar y judicialización. En ese grupo, hubo 1 niño y 2 progenitores muertos por causa violenta. No hubo ninguna muerte en el grupo no expuesto.Conclusiones: Los niños expuestos a cocaína y/o cannabis prenatalmente tuvieron menor número de controles de salud y mayor frecuencia de consultas en Emergencias e Internaciones. Presentaron, además, mayor frecuencia de enfermedad pulmonar obstructiva crónica, cambios de hogar, judicialización y muertes violentas en el grupo familiar directo.


Cocaine and/or cannabis use during pregnancy is a growing problem of relevance for global public health. Exposed children may have a wide range of perinatal complications, but there is little knowledge on their course.Objective: To describe and compare the health trajectories of children prenatally exposed and unexposed to cocaine and/or cannabis over 4 years.Methods: Retrospective, cohort study with a double control group. Exposed children were detected through a urine test by the Department of Neonatology of a public hospital between 2009 and 2013. Results: A total of 29 exposed children and 58 unexposed children were assessed. The most common drugs detected in the exposed group were cocaine and cannabis. Most mothers were poly-drug users. The exposed group showed significant differences in relation to a lower frequency of health checkups (p < 0.0001) and a higher number of visits to the emergency department (p = 0.0295) and hospitalizations (p = 0.007), mainly due to respiratory conditions. In addition, they had a greater rate of chronic obstructive pulmonary disease, changes of home, and legal interventions. In this group, 1 child and 2 parents had a violent death. No deaths were reported in the unexposed group. Conclusions: Children prenatally exposed to cocaine and/or cannabis had a lower number of health checkups and a higher number of visits to the emergency department and hospitalizations. Besides, they showed a greater rate of chronic obstructive pulmonary disease, changes of home, legal interventions, and violent deaths in the direct family group.

Resumen La cocaína es una sustancia psicoactiva ampliamente utilizada de forma recreativa. El uso indiscriminado de esta sustancia genera múltiples consultas a los servicios de emergencias, principalmente debido a sus efectos cardiovasculares. Los mecanismos fisiopatológicos actúan principalmente en sistema nervioso simpático, cardiomiocito, plaquetas, canales de sodio y potasio, entre otros. El objetivo principal de esta revisión es demostrar los efectos cardiovasculares asociados al uso de la cocaína.


Abstract Cocaine is a psychoactive substance widely used recreationally. The indiscriminate use of this substance generates multiple consultations to the emergency services, mainly due to its cardiovascular effects. The physio pathological mechanisms act mainly in sympathetic nervous system, cardiomyocyte, platelets, sodium and potassium channels, among others. The main objective of this review is to demonstrate the cardiovascular effects associated with the use of cocaine.

ABSTRACT

Uma das principais dificuldades enfrentadas na dependência à cocaína está relacionada aos sintomas de abstinência, como ansiedade, desejo e irritabilidade. Estes efeitos podem durar meses ou anos após a interrupção do consumo prolongado, fazendo com que o indivíduo volte a procurá-la. Os efeitos recompensadores da cocaína levam a alterações neurobiológicas do sistema mesocorticolímbico dopaminérgico, que se origina na área tegmental ventral e se projeta para o núcleo accumbens, e córtex pré-frontal, áreas intimamente ligadas ao desenvolvimento da dependência. Esses neurônios dopaminérgicos recebem estímulos dos neurônios colinérgicos que contribuem para os aspectos cognitivos da dependência. Devido à complexidade neurobilógica envolvida durante a abstinência, pouco se sabe sobre as alterações no sistema colinérgico muscarínico durante este período no encéfalo, objetivo deste estudo. Para tal, camundongos machos adultos Swiss-Webster foram submetidos à cocaína em padrão agudo em binge (3×30 mg/kg/dia) e cronicamente por escalonamento de dose em binge por 14 dias (3×15 mg/kg/dia nos dias 1-4; 3×20 mg/kg/dia nos dias 5-8; 3×25 mg/kg/dia nos dias 9-12; e 3×30 mg/kg/dia nos dias 13 e 14). A atividade locomotora de cada animal foi avaliada em campo aberto (CA), onde permaneceram no aparato por 60 minutos entre cada administração. Após o período de exposição os animais permaneceram 14 dias em abstinência, a fim de avaliar a ansiedade no labirinto em cruz elevado (LCE). Em seguida os animais foram eutaniasiados, sendo o córtex pré-frontal (CPF), o estriado e o hipocampo dissecados e armazenados a -80ºC para a análise dos receptores dopaminérgicos D1 e D2, receptores colinérgicos muscarínicos M1, M2, M3, M4 e M5 (mAChRs) e moléculas colinérgicas (acetilcolinesterase, AChE; colina acetiltransferase, ChAT e transportador vesicular de acetilcolina, VAChT) por Western Blotting (n=6). Os resultados comportamentais mostraram maior atividade locomotora nos animais tratados com cocaína no tratamento agudo ou crônico, quando comparado ao basal. Mais ainda, a sensibilização comportamental foi detectada a partir do segundo dia de administração de cocaína. No teste de LCE, realizado 14 dias após a interrupção da administração de cocaína, não foi observada diferença estatística entre os animais previamente expostos à cocaína e grupo controle. No CPF observou-se diminuição de D2R, M1 mAChRs e aumento M2 e M4 mAChRs no tratamento agudo; no tratamento crônico houve diminuição de M1 e M5 mAChRs e ChAT. No estriado observou-se aumento de D1R, M1 e M2 mAChRs, ChAT no tratamento agudo; e aumento D1R, VAChT, ChAT e diminuição D2R, M1 e M2 mAChRs no tratamento crônico. Já no hipocampo observou-se aumento de D1R, D2R, M2 mAChRs, VAChT e diminuição M1 mAChRs no tratamento agudo; e aumento de D1R, VAChT e diminuição D2R, M1 mAChRs no tratamento crônico. Nossos resultados mostram envolvimento de processo de neuroplasticidade, tanto no sistema dopaminérgico quanto no colinérgico muscarínico, em ambos os protocolos utilizados, mesmo após 14 dias de abstinência


Una de las dificultades enfrentadas en la dependencia de cocaína son los síntomas de abstinencia, como ansiedad, deseo y irritabilidad. Estos efectos pueden durar meses o años después de la interrupción del consumo prolongado, haciendo que el individuo vuelva a consumirlo. Los efectos recompensadores de la cocaína causa alteraciones neurobiológicas del sistema mesocorticolímbico dopaminérgico, que se origina en el área tegmental ventral y se proyecta hacia el núcleo accumbens y córtex pré-frontal, áreas íntimamente ligadas al desenvolvimiento de la dependencia. Esas neuronas dopaminérgicas reciben estímulos de neuronas colinérgicas la cual contribuyen para los aspectos cognitivos de la dependencia. Debido a la complejidad neurobiológica involucrada durante la abstinencia, poco se sabe sobre las alteraciones del sistema colinérgico muscarínico durante este periodo en el encéfalo, objetivo de este estudio. Por tanto, ratones adultos macho Swiss-Webster fueron sometidos a cocaína en dosis padrón agudo en binge (3×30 mg/kg/día) y crónicamente por escalonamiento de dosis en binge por 14 días (3×15 mg/kg/día en los días 1-4; 3×20 mg/kg/día en los días 5-8; 3×25 mg/kg/día en los días 9-12; y 3×30 mg/kg/día en los días 13 e 14). La actividad locomotora de cada animal fue evaluada en el test de campo abierto (CA), donde permanecieron por 60 minutos entre cada administración. Después del periodo de exposición los animales permanecieron 14 días de abstinencia, a fin de evaluar la ansiedad en el labirinto de cruz elevado (LCE). En seguida los animales fueron eutanasiados, donde el córtex pré-frontal (CPF), estriado y hipocampo fueron disecados y almacenados a -80ºC para analizar los receptores dopaminérgicos D1 e D2, receptores colinérgicos muscarínicos M1, M2, M3, M4 y M5 (mAChRs) y moléculas colinérgicas (acetilcolinesterasa, AChE; colina acetiltransferasa, ChAT y transportador vesicular de acetilcolina, VAChT) por Western Blotting (n=6). Los resultados comportamentales mostraron mayor actividad locomotora en los animales tratados con cocaína en tratamiento agudo y crónico, comparado al control. Por otra parte, la sensibilización comportamental fue detectado a partir de segundo día de administración de cocaína. En la prueba de LCE, realizado después de 14 días de interrupción de la administración de cocaína, no fue observado diferencia estadística entre los animales previamente expuestos a la cocaína y el grupo control. En CPF se observó disminución de D2R, M1 mAChRs y aumento de M2 y M4 mAChRs en tratamiento agudo; en el tratamiento crónico mostro disminución de M1 y M5 mAChRs y ChAT. En el estriado se observó aumento de D1R, M1 y M2 mAChRs, ChAT en el tratamiento agudo; aumento D1R, VAChT, ChAT y disminución de D2R, M1 y M2 mAChRs en el tratamiento crónico. Por último, en el hipocampo se observó aumento de D1R, D2R, M2 mAChRs, VAChT y disminución M1 mAChRs en el tratamiento agudo; aumento de D1R, VAChT y disminución D2R, M1 mAChRs en el tratamiento crónico. Nuestros resultados muestran envolvimiento de procesos de neuroplasticidad, tanto en el sistema dopaminérgico como el sistema colinérgico muscarínico, en ambos protocolos utilizados, después de 14 días de abstinencia

ABSTRACT

O abuso de drogas atinge aproximadamente 35 milhões de pessoas em todo planeta, sendo um problema alarmante em decorrência de graves danos à saúde, como a dependência química e intoxicações fatais. No Brasil, o número de usuários tem crescido principalmente para o consumo de produtos da Cannabis e cocaína, drogas amplamente consumidas, inclusive entre mulheres em período gestacional, trazendo à tona um novo grupo de risco. A exposição gestacional a drogas de abuso está diretamente relacionada a malformações fetais e complicações de saúde para mãe e bebê nos períodos pré- e pós-natal. Tradicionalmente, a avaliação toxicológica da exposição é realizada pela detecção da droga parental e de seus produtos de biotransformação em matrizes materno-fetais por meio de métodos bioanalíticos. Entretanto, estes ensaios não fornecem informações acerca dos impactos fisiológicos ocasionados pela exposição, deixando uma lacuna no que tange às informações sobre os mecanismos e moléculas subjacentes envolvidos em processos de toxicidade. Desse modo, o desenvolvimento de análises toxicológicas mais robustas utilizando tecnologia de ponta, que possam comprovar o uso drogas e também elucidar aspectos de toxicidade é de suma importância, pois auxiliam na compreensão do impacto biológico relativo à exposição humana a xenobióticos. Neste trabalho foram desenvolvidos ensaios bioanalíticos, utilizando o tecido do cordão umbilical para a avaliação da exposição in utero à canabinoides. Foi desenvolvido e validado método QuECheRS adaptado como preparo de amostra, no qual etapas simultâneas de extração e hidrólise alcalina de canabinoides são alcançadas, utilizando cromatografia em fase gasosa acoplada a espectrômetro de massas para detecção de delta-9-tetraidrocanabinol (THC), canabinol (CBN), 11-hidroxi-delta-9-tetraidrocanabinol (11-OHTHC) e 11-nor-9-carboxi-tetrahidrocanabinol (THC-COOH). Também foram desenvolvidas metodologias utilizando LC-MS/MS e Trapped Ion Mobility Mass Spectrometry para análise de proteoma de cordão umbilical humano em diferentes regiões, no intuito de identificar biomarcadores proteicos relativos à fetotoxicidade do uso de drogas na gravidez. Até o presente momento, QuECheRS é utilizado pela primeira vez como abordagem bioanalítica para avaliação de drogas ilícitas em matrizes teciduais materno-fetais e mostrou-se satisfatório para detecção de produtos da Cannabis. Nos ensaios proteômicos, foram identificados potenciais biomarcadores de fetotoxicidade, como as moléculas ACTA 2, Collagen alpha-1 (XVIII), SMC1A, KNL1, KMT2A, em tecidos expostos à Cannabis e/ou cocaína. Tais macromoléculas estão correlacionadas a malformações embriogênicas e complicações de saúde na vida intra-uterina. As metodologias desenvolvidas neste trabalho podem ser úteis para uma melhor avaliação da toxicidade do uso de drogas na gravidez, fornecendo novas pistas sobre a exposição e/ou efeitos tóxicos significativos considerados na avaliação de risco


Drug abuse affects approximately 35 million people worldwide and can be considered a significant burden on society due to severe health problems, e.g. drug addiction and fatal poisonings. In Brazil, the number of users has been growing related to Cannabis and cocaine products, drugs widely used, including among women in gestational period, bringing up a new risk group. Gestational exposure to drugs of abuse is directly related to fetal malformations and health complications for mother and babies in the pre- and postnatal periods. Traditionally, toxicological assessment of exposure is performed by detecting the parent drug and its biotransformation products in maternal-fetal matrices using bioanalytical methods. However, these assays do not provide information about the physiological impacts caused by exposure, leaving a lack of information about the pathways and molecules involved in toxicity processes. Thus, the development of robust toxicological analyzes using cutting-edge technologies in order to prove drug use and also elucidate aspects of toxicity is very important, as they help in understanding the biological impact of human exposure to xenobiotics. Herein, bioanalytical methods using umbilical cord tissue to assess in utero exposure to cannabinoids were developed. A QuECheRS method was developed fully validated as a sample preparation technique for simultaneous extraction and alkaline hydrolysis of cannabinoids, using gas chromatography coupled to mass spectrometry to detect the analytes delta-9-tetrahydrocannabinol (THC), cannabinol (CBN), 11-hydroxydelta-9-tetrahydrocannabinol (11-OH-THC) and 11-nor-9-carboxy-tetrahydrocannabinol (THC-COOH). LC-MS/MS based proteomics and Trapped Ion Mobility Mass Spectrometry were also developed in order to identify protein biomarkers related to fetotoxicity of drug use in pregnancy. Our works represents the first use of QuECheRS for evaluation of illicit drugs in maternal-fetal tissue and was suitable for detection of Cannabis products. In the proteomic assays, potential biomarkers of fetotoxicity were identified in the exposed tissues, such as ACTA 2, Collagen alpha-1 (XVIII), SMC1A, KNL1, KMT2A. These proteins are related to embryogenic malformations and health complications in intrauterine life. The methodologies developed in this project may be useful for a better assessment of the toxicity of drug use in pregnancy, providing new clues about exposure and/or significant toxic effects that should be considered in the risk assessment

Resumo O uso de drogas traz diversos malefícios para a saúde bucal, aumentando a prevalência das doenças cárie dentária e periodontal, representando um sério problema de saúde pública. Este estudo objetivou avaliar a prevalência das doenças cárie dentária e periodontal em usuários de drogas institucionalizados. Estudo epidemiológico, transversal, censitário e analítico realizado com usuários de drogas institucionalizados no período de agosto de 2016 a junho de 2017. Examinadores padronizados aplicaram um formulário para coleta de dados sociodemográficos e consumo de drogas. A cárie dentária foi avaliada através do índice CPO-D e a doença periodontal através dos indicadores clínicos: sangramento à sondagem, índice de placa, índice gengival, profundidade de sondagem e nível de inserção clínica. Foram avaliados 112 indivíduos do sexo masculino, média de idade de 36,7 anos (Dp±11,5). A droga mais consumida foi o álcool (92,0 %) por cerca de 19,4 anos (Dp±11.08), seguido do tabaco (70,95%) em média por 18,0 anos (Dp±11,66), maconha (65,2%) por 13,7 anos (Dp±8,22), cocaína (53,3%) por 8,5 anos (Dp±6,00) e crack (50,0%) por 11,1 anos (Dp±7,18). Conclui-se que a prevalência da doença de cárie foi 97,3% e da doença periodontal 51,5%. A população estudada apresentou alta prevalência de cárie dentária e doença periodontal e houve correlação entre o tempo de uso do álcool e tabaco com a doença periodontal e com maior CPO-D.

Abstract Drug use causes several health hazards to oral health, increasing the prevalence of dental caries and periodontal diseases, representing a serious public health problem. This study aimed to evaluate the prevalence of dental and periodontal caries in institutionalized drug users. It is an epidemiological, cross-sectional, census-based, analytical study with institutionalized drug users, carried out from August 2016 to June 2017. Standardized examiners applied a form to collect sociodemographic data and on drug consumption. Dental caries were evaluated through the DMFT index and periodontal disease through clinical indicators: bleeding at probing, plaque index, gingival index, probing depth and level of clinical insertion. A total of 112 males, mean age 36.7 years (Dp ± 11.5) were evaluated. The most commonly used drug was alcohol (92.0%) for about 19.4 years (Dp ± 11.08), followed by tobacco (70.95%) on average for 18.0 years (Dp ± 11.66), marijuana (65.2%) for 13.7 years (Dp ± 8.22), cocaine (53.3%) for 8.5 years (Dp ± 6.00) and crack (50.0%) for 11.1 year (Dp ± 7.18). It is concluded that the prevalence of caries was 97.3% and of periodontal disease 51.5%. The studied population presented high prevalence of dental caries and periodontal disease and there was a correlation between alcohol and tobacco use time with periodontal disease and with higher DMFT.

Introducción: El consumo de cocaína durante la gestación gatilla isquemia, muerte y licuefacción celular en el cerebro fetal, consolidando en la infancia grados variables de retraso mental. El presente estudio busca identificar mediante test de drogas en orina los recién nacidos (RN) expuestos a cocaína en el embarazo y describir el procedimiento clínico y social a seguir. Metodología: Estudio de cohorte prospectivo enero 2016 y enero 2018 en RN con exposición antenatal a cocaína, Unidad de Neonatología del Hospital Clínico San Borja Arriarán. Resultados: Se estudió a 64 RN con test en orina positivo a cocaína. El 42% fue pequeño para la edad gestacional, 33% tenía microcefalia. Se encontraron malformaciones en sistema nervioso y vías urinarias, trastornos del ritmo cardíaco e hipoacusia. Solo 32,8% de las madres controló su embarazo y 52% rechazó la rehabilitación. Servicio Social interpuso medidas de protección a los RN e instó a las madres a programas de rehabilitación. El 12,5% de los RN no tenía familia de apoyo y debió ser derivado a instituciones gubernamentales. Conclusiones: Las consecuencias de la exposición a cocaína antenatal en el RN son devastadoras. Este trabajo permite orientar la pesquisa, estudio y pasos legales a seguir con los RN afectados y sus madres.


Introduction: The consumption of cocaine during pregnancy triggers events such as ischemia, death and cell liquefaction in the fetal brain, consolidating varying degrees of intellectual disability. This study proposed to identify by urine drug test the newborns (NB) with antenatal exposure to and describe the clinical and social procedure to follow with them and their mothers until neonatal discharge. Methodology: Prospective cohort study, conducted in RN who met criteria for risk of antenatal exposure to cocaine, Neonatology Unit of the San Borja Arriaran Clinical Hospital between January 2016 -2018. Results: Antenatal exposure to cocaine was confirmed on 64 NB. Forty-two percent of them were small for gestational age and 33% had microcephaly. Malformations were found in the nervous system urinary tract, as well as disorders in the rhythm of the heart and loss of hearing. Only 32% of mothers controlled her pregnancy, none of them was derived to the secondary. Social Services implemented all the NB protective measures in place and urged mothers to participate in rehabilitation programs. Fifty-two percent rejected rehabilitation and 12.5% of the NB have not family support and had to be referred to government institutions. Conclusions: The consequences of exposure to antenatal cocaine in the NB are devastating. This work allows orienting the research with the NB and showing the legal steps should be taken with the RN and their mothers.

ABSTRACT

As intoxicações decorrentes do uso de drogas de abuso representam atualmente um grave problema para a saúde pública. Dentre os principais agentes envolvidos, destaca-se a cocaína. Ela se tornou uma das drogas mais consumidas ao redor do mundo, sendo um dos principais motivos de atendimentos em pronto-socorro (PS) devido ao uso de substâncias ilícitas. Seu uso ocorre principalmente em associação com bebida alcóolica. Existem poucos estudos realizados que relacionem a concentração sanguínea de cocaína e a gravidade das manifestações clínicas em populações que a utilizam como droga de abuso, e que envolvam pacientes atendidos em PS. O objetivo do presente estudo foi verificar a possível relação entre concentração sanguínea de cocaína e cocaetileno (produto da interação de cocaína com etanol) com a gravidade das manifestações clínicas apresentadas por pacientes com hipótese diagnóstica de intoxicação por cocaína. As concentrações sanguíneas foram determinadas por cromatografia líquida de alta eficiência (HPLC) e a gravidade das manifestações clínicas foi avaliada através do Stimulant Intoxication Score (SIS). Dos 81 pacientes incluídos no estudo 77,8% são homens com idade média de 32,5 anos ± 8,5 e SIS médio de 3,4 ± 2,5. Do total de pacientes incluídos no estudo 20 (24,7%) tiveram resultados positivos em sangue para os analitos de interesse, sendo a concentração sanguínea média de cocaína igual a 0,34 µg/mL ± 0,45 e de cocaetileno igual a 0,38 µg/mL ± 0,34. A concentração sanguínea de cocaína e cocaetileno não se mostrou informação útil para tratamento e prognóstico dos pacientes, porém a detecção no sangue destas substâncias no momento do atendimento, independentemente de sua concentração, pode ser um indicador de gravidade, mostrando que quaisquer concentrações destas substâncias devem ser consideradas potencialmente tóxicas. A aplicação do score SIS revelou-se como importante alternativa capaz de predizer a gravidade dos pacientes atendidos devido a intoxicação por cocaína de maneira rápida e simplificada


Currently, poisoning resulting from the abuse of drug represents a serious problem for public health. Among the main agents involved, cocaine stands out. It became one of the most abused drugs around the world, being one of the main reasons for visits to the emergency room due to the use of illicit substances. The use of cocaine is primarily in combination with alcoholic beverages. There are few studies that relate cocaine blood concentration and the severity of its clinical manifestations in patients attended in the Emergency Room. The aim of the present study was to verify the possible relationship between the blood concentration of cocaine and cocaethylene (product of the interaction of cocaine with ethanol) with the severity of the clinical manifestations presented by patients with cocaine intoxication. Blood levels were measured by high-performance liquid chromatography (HPLC) and the severity of clinical manifestations was assessed using the Stimulant Intoxication Score (SIS). Of the 81 patients included in the study, 77.8% were men with a mean age of 32.5 years ± 8.5 and mean of SIS 3.4 ± 2.5. From the total of patients included in the study 20 (24.7%) had positive blood results for the analytes of interest, being the mean blood concentration of cocaine equal to 0,34 µg/mL ± 0,45 and of cocaethylene equal to 0,38 µg/mL ± 0,34. The blood concentration of cocaine and cocaethylene has not been shown to be useful information for the treatment and prognosis of patients, but blood levels of these substances at the time of treatment, regardless of their concentration, may be an indicator of severity, showing that any concentrations of these substances should be considered as potentially toxic. The application of the SIS score proved to be an important alternative capable of predicting the severity of the patients attended due to cocaine intoxication in a fast and simplified way

Abstract: We report a 42-year-old cocaine addicted female patient referred for evaluation of hard palate ulceration resulting in oro-sinus communication with difficulties in swallowing and phonation, an rhino-sinusitis. Acrylic and removable silicone prosthesis was prescribed to relieve severe functional disorders. It is essential that the patient permanently abandons cocaine use to perform surgical reconstruction.

ABSTRACT

El consumo de clorhidrato y pasta base de cocaína en Uruguay tiene una prevalencia de vida del 6,9% y 0,7% respectivamente. Las complicaciones tóxicas secundarias al consumo dependen de la concentración del alcaloide así como de la presencia de adulterantes. El levamisol, antihelmíntico veterinario, reconocido adulterante de la cocaína en Estados Unidos (2003), fue detectado en Uruguay en el año 2013. Este adulterante genera complicaciones tales como neutropenia, vasculitis cutánea, glomerulonefritis, hemorragia pulmonar y leucoencefalopatía. Se describen los tres primeros casos clínicos de complicaciones por levamisol como adulterante de cocaína reportados al Centro de Información y Asesoramiento Toxicológico. Los pacientes eran consumidores crónicos con edades entre los 35 y 40 años. En los tres casos se objetivó un púrpura retiforme con centro necrótico asociado a anticuerpos ANCA positivos con tendencia a presentarse en lóbulos de orejas, mejillas y extremidades, tal como se reporta en las vasculitis por levamisol. Se reportó neutropenia asociada en un caso. Los tres pacientes presentaron anemia. Dos casos presentaron falla renal aguda. El tratamiento principal fue el cese del consumo, lo que produjo una reversión completa de las complicaciones. Todos requirieron debridación o injertos de piel, o ambos. Se discuten y analizan las complicaciones mencionadas así como el riesgo que conlleva la reexposición y los tratamientos propuestos para las mismas, tales como los factores de crecimiento de granulocitos, antibióticos de amplio espectro y los corticoides. Se requiere un alto índice de sospecha para vincular estas manifestaciones clínicas a la presencia de levamisol como adulterante de la cocaína.


Consumption of cocaine hydrochloride and cocaine base paste (CBP) in Uruguay has a lifetime prevalence of 6.9% and 0.7% respectively. Complications associated to toxic side effects of cocaine abuse depend on the concentration of the alkaloid, as well as on the presence of adulterants. Levamisole, a veterinary anthelmintic, a well know cocaine adulterant in the US was identified in Uruguay in 2013. This adulterant results in complications such as neutropenia, skin vasculitis, glomerulonephritis and leukoencephalopathy. The study describes the first three clinical cases of complications resulting from cocaine cut with levamisole reported to the Toxicology Center for Information and Counseling. The patients included were chronic cocaine abusers between 35 and 40 years old. Retiform purpura was detected in the three cases with a necrotic center associated to ANCA antibodies, with a tendency to appear in the earlobes, cheeks, and limbs, as it is reported in levamisole associated vasculitis. Neutropenia was reported in one case. The three patients had anemia. Two of the cases presented acute kidney failure. The main treatment was interruption of consumption, what resulted in a complete reversion of complications. All patients required debridement or skin grafts, or both. The above mentioned complications are discussed and analyzed, as well as the risk re-exposure and the suggested treatments involve, as the granulocytes growth factors, broad spectrum antibiotics and corticoids. A high degree of suspicion is required to associate these clinical manifestations to the presence of levamisole as a cocaine adulterant.

O levamisol, anti-helmíntico veterinário, um conhecido adulterante da cocaína nos Estados Unidos (2003), foi detectado no Uruguai em 2013. Este adulterante gera complicações tais como neutropenia, vasculite cutânea, glomerulonefrite, hemorragia pulmonar e leucoencefalopatia. Os três primeiros casos clínicos de complicações por levamisol como adulterante de cocaína informados ao Centro de Información y Asesoramiento Toxicológico são descritos. Os pacientes eram consumidores crônicos com idades entre 35 e 40 anos. Nos três casos se observou púrpura retiforme com centro necrótico associado a anticorpos ANCA positivos com tendência a apresentação nos lóbulos das orelhas, bochechas e extremidades, como estão descritas nas vasculites por levamisol. Em um caso foi observada neutropenia. Três pacientes apresentaram anemia e dois falha renal aguda. O tratamento principal foi a cessação do consumo com reversão total das complicações. Foi necessário realizar debridação ou enxertos de pele, ou ambos em todos os casos. As complicações mencionadas bem como o risco vinculado à reposição e os tratamentos propostos para as mesmas, como por exemplo os fatores de crescimento de granulócitos, antibióticos de amplio espectro e corticoides, são discutidos e analisados. É necesssário um alto índice de suspeita para vincular estas manifestações clínicas à presença de levamisol como adulterante da cocaína.

Contexto: O número de usuários de drogas ilícitas é estimado globalmente em 210 milhões de pessoas ao ano, das quais pelo menos 200.000 morrem em decorrência do uso. O consumo de drogas afeta não apenas o usuário, mas também a família, os amigos, os colegas de trabalho e a comunidade. Objetivo: O presente estudo traçou um perfil sociodemográfico de trabalhadores em tratamento para recuperação da dependência química na região de Campinas (SP). Método: Estudo transversal exploratório com dados colhidos por meio de questionário autoaplicado e anônimo em centros de recuperação para tratamento de dependência química no município de Campinas. A pesquisa foi conduzida entre julho de 2011 e julho de 2012 e analisou as condições de saúde e trabalho. A população estudada constituía-se de trabalhadores que estiveram em acompanhamento por mais de 30 dias, totalizando 200 pessoas. Resultados: A faixa etária de 18 a 54 anos representou 87,8% da amostra, com predominância de participantes com ensino médio completo e que desenvolviam alguma atividade remunerada, sendo que 54,5% estavam empregados formalmente, 48,4% com contratos inferiores a um ano, e 42,1% com faixa salarial abaixo de R$ 1.020,00. Conclusão: Este estudo apresenta informações pouco exploradas em outras pesquisas e com potencial para aprimorar as intervenções relacionadas à prevenção, ao tratamento e à reabilitação de trabalhadores em situação semelhante.


Context: The number of illicit drug users is estimated at 210 million people a year worldwide, of which at least 200,000 die as a consequence of the use. Drug use affects not only the user but also the family, friends, coworkers, and the community. Objective: This study outlines a sociodemographic profile of workers undergoing treatment for chemical dependency recovery in Campinas (SP). Method: Cross-sectional, exploratory study with data collected in rehabilitation centers for chemical dependency treatment in Campinas by means of a self-administered and anonymous questionnaire. The research was conducted between July 2011 and July 2012 and focused on the analysis of health and work conditions. The study included workers who were followed up for more than 30 days (n=200). Results: 87.8% of the population were aged 18­54 years, most of them had completed high school, had a remunerated activity, and of which 54.5% were formally employed, 48.4% with less than one year contracts and 42.1% with salary range below R$1,020.00 (Brazilian real). Conclusion: This study provides information not explored in other researches and has a potential value related to the prevention, treatment, and rehabilitation of workers in similar conditions.

Background: Consumption of illicit drugs (ID) has been associated with an increased risk of acute myocardial infarction (AMI). There is limited national evidence about the impact of substance use over the clinical presentation, management and outcomes of AMI patients. Aim: To describe the prevalence of ID consumption in patients within the Chilean Registry of Myocardial Infarction (GEMI), comparing clinical characteristics, management and outcome according to consumption status. Material and Methods: We reviewed data from the GEMI registry between 2001 and 2013, identifying 18,048 patients with AMI. The sample was stratified according to presence or absence of previous ID consumption, comparing different demographic and clinical variables between groups. Results: Two hundred eighty five patients (1.6%) had history of ID consumption (cocaine in 66%, cannabis in 35% and central nervous system stimulants in 24.0%). Compared with non-users, ID consumers were younger, predominantly male and had a lower prevalence of cardiovascular risk factors, except for tobacco smoking (86.3% and 42.5% respectively, p < 0.01). Among consumers, there was a higher percentage of ST segment elevation (85.2% and 67.8% respectively, p < 0.01) and anterior wall AMI (59.9 and 49.5% respectively, p = 0.01). Additionally, they had a higher rate of primary angioplasty (48.8% and 25.5% respectively, p < 0.01). There was no difference in hospital mortality between groups when stratified by age. Conclusions: A low percentage of patients with AMI had a previous history of ID consumption in our national setting. These patients were younger and had a greater frequency of ST segment elevation AMI, which probably determined a more invasive management.

Levamisole has been increasingly used as an adulterant of cocaine in recent years, emerging as a public health challenge worldwide. Levamisole-associated toxicity manifests clinically as a systemic vasculitis, consisting of cutaneous, hematological, and renal lesions, among others. Purpura retiform, cutaneous necrosis, intravascular thrombosis, neutropenia, and less commonly crescentic nephritis have been described in association with anti-neutrophil cytoplasmic antibodies (ANCAs) and other autoantibodies. Here we report the case of a 49-year-old male who was a chronic cocaine user, and who presented spontaneous weight loss, arthralgia, and 3 weeks before admission purpuric skin lesions in the earlobes and in the anterior thighs. His laboratory tests on admission showed serum creatinine of 4.56 mg/dL, white blood count 3,800/μL, hemoglobin 7.3 g/dL, urinalysis with 51 white blood cells/μL and 960 red blood cells/μL, and urine protein-to-creatinine ratio 1.20. Serum ANCA testing was positive (>1:320), as well as serum anti-myeloperoxidase and anti-proteinase 3 antibodies. Urine toxicology screen was positive for cocaine and levamisole, with 62.8% of cocaine, 32.2% of levamisole, and 5% of an unidentified substance. Skin and renal biopsies were diagnostic for leukocytoclastic vasculitis and pauci-immune crescentic glomerulonephritis, respectively. The patient showed a good clinical response to cocaine abstinence, and use of corticosteroids and intravenous cyclophosphamide. Last serum creatinine was 1.97 mg/dL, white blood cell count 7,420/μL, and hemoglobin level 10.8 g/dL. In levamisole-induced systemic vasculitis, the early institution of cocaine abstinence, concomitant with the use of immunosuppressive drugs in severe cases, may prevent permanent end organ damage and associate with better clinical outcomes.

ABSTRACT

The prevalence of drug-associated toxic encephalopathy is unknown, but it is an uncommon condition. Toxic leukoencephalopathy was described associated with heroin consumption, it has been less commonly described with the use of cocaine and there are no reports of its association with consumption pasta base of cocaine (PBC). We report two females aged 31 years and a male aged 19 years, consumers of PBC who developed a fatal toxic leukoencephalopathy. They initiated their disease with severe and persistent headache, sequential focal neurologic deficits and a progressive impairment of consciousness that culminated with their death. Laboratory parameters such as blood count, cerebrospinal fluid analyses or infectious biological indices were normal. MRI showed multifocal lesions in brain white matter of both hemispheres confirming the leukoencephalopathy. There was no response to the use of methylprednisolone.

AbstractObjective: To evaluate HRCT scans of the chest in 22 patients with cocaine-induced pulmonary disease.Methods: We included patients between 19 and 52 years of age. The HRCT scans were evaluated by two radiologists independently, discordant results being resolved by consensus. The inclusion criterion was an HRCT scan showing abnormalities that were temporally related to cocaine use, with no other apparent causal factors.Results:In 8 patients (36.4%), the clinical and tomographic findings were consistent with "crack lung", those cases being studied separately. The major HRCT findings in that subgroup of patients included ground-glass opacities, in 100% of the cases; consolidations, in 50%; and the halo sign, in 25%. In 12.5% of the cases, smooth septal thickening, paraseptal emphysema, centrilobular nodules, and the tree-in-bud pattern were identified. Among the remaining 14 patients (63.6%), barotrauma was identified in 3 cases, presenting as pneumomediastinum, pneumothorax, and hemopneumothorax, respectively. Talcosis, characterized as perihilar conglomerate masses, architectural distortion, and emphysema, was diagnosed in 3 patients. Other patterns were found less frequently: organizing pneumonia and bullous emphysema, in 2 patients each; and pulmonary infarction, septic embolism, eosinophilic pneumonia, and cardiogenic pulmonary edema, in 1 patient each.Conclusions: Pulmonary changes induced by cocaine use are varied and nonspecific. The diagnostic suspicion of cocaine-induced pulmonary disease depends, in most of the cases, on a careful drawing of correlations between clinical and radiological findings.


ResumoObjetivo:Avaliar achados em TCAR de tórax de 22 pacientes com doença pulmonar induzida pelo uso de cocaína.Métodos:Foram incluídos pacientes com idades variando de 19 a 52 anos. As TCAR foram avaliadas por dois radiologistas, de forma independente, e os casos discordantes foram resolvidos por consenso. O critério de inclusão foi a presença de anormalidades na TCAR temporalmente relacionadas ao uso de cocaína, sem outros fatores causais justificáveis.Resultados:Oito pacientes (36,4%) apresentavam quadro clínico-tomográfico compatível com "pulmão de crack", e esses casos foram estudados separadamente. Os achados tomográficos predominantes nesse subgrupo de pacientes foram opacidades em vidro fosco, em 100% dos casos; consolidações, em 50%; e sinal do halo, em 25%. Em 12,5% dos casos, observou-se espessamento septal liso, enfisema parasseptal, nódulos centrolobulares e padrão de árvore em brotamento. Dentre os outros 14 pacientes (63,6%), observou-se barotrauma em 3 casos, apresentando-se como pneumomediastino, pneumotórax, e hemopneumotórax, respectivamente. Talcose foi diagnosticada em 3 pacientes, caracterizada como massas conglomeradas peri-hilares, distorção arquitetural e enfisema. Outros padrões encontrados com menor frequência foram pneumonia em organização e enfisema bolhoso, observados em 2 pacientes cada; e infarto pulmonar, embolia séptica, pneumonia eosinofílica e edema pulmonar cardiogênico, em 1 paciente cada.Conclusões:As alterações pulmonares induzidas pelo uso de cocaína são múltiplas e inespecíficas, e sua suspeita diagnóstica depende, na maioria dos casos, de uma cuidadosa correlação clínico-radiológica.
